Job Title: Transitional Living Program (TLP) Case Manager
Location: Medford, Oregon (in-person)
Full-time (40 hours/week)
Pay: $21.00 - $30.00 per hour

Job description

Your Role:

As a Transitional Living Program (TLP) Case Manager for runaway and homeless youth, you will be responsible for guiding youth (ages 16-22) in our Transitional Living Program to achieve stability and independence. Using a trauma-informed, person-centered approach, you will provide intensive case management, life skills development, and resource navigation to support long-term success. You’ll also collaborate with community partners, landlords, and service providers to remove barriers to stable housing and employment.

Key Responsibilities:

  • Case Management:

  • Conduct comprehensive assessments to determine each youth's needs and barriers to stability, such as housing, education, employment, and mental health services.

  • Develop individualized transition plans focusing on achieving housing stability, educational success, and employment readiness.

  • Provide ongoing support and mentorship in life skills, including budgeting, personal care, communication, and job readiness.

  • Help youth build emotional resilience and problem-solving skills to handle challenges in their path toward self-sufficiency.

  • Transitional Housing Support:

  • Oversee youth's participation in the Transitional Living Program, ensuring they meet program goals and requirements.

  • Assist youth with securing and maintaining safe, affordable housing as they prepare to exit the program.

  • Facilitate connections to housing resources, including rental assistance programs, utility support, and landlord-tenant advocacy.

  • Provide crisis intervention and conflict resolution support as needed.

  • Resource Navigation and Advocacy:

  • Collaborate with local shelters, schools, counseling services, and health providers to connect youth with essential resources.

  • Advocate on behalf of youth during landlord and service provider interactions to ensure they receive fair treatment.

  • Ensure youth are connected to educational opportunities, job training, and employment resources to build self-sufficiency.

  • Community Outreach and Partnership:

  • Cultivate relationships with landlords, property managers, and employers to expand housing and job opportunities for youth.

  • Promote Maslow Project’s services to community partners, attending relevant meetings and coordinating outreach efforts.

  • Participate in community initiatives like the annual Point in Time Count and other events focused on identifying and assisting runaway and homeless youth.

  • Documentation and Reporting:

  • Maintain accurate and up-to-date case notes, housing agreements, and service plans.

  • Track youth progress and outcomes using our internal database, ensuring compliance with program requirements.

  • Report data on youth participation, housing placements, and assistance expenses.

Experience and Qualifications:

  • Education: High School Diploma, GED, or College Degree (2 or 4 years) in a related field (Social Work, Human Services, etc.).

  • Experience: Minimum 2 years of experience working with homeless youth, runaway youth, or in transitional living programs. Experience in case management, crisis intervention, or social services preferred.

  • Certifications: Community Health Worker (CHW), Traditional Health Worker (THW), or Qualified Mental Health Associate (QMHA-I) certification is preferred or in progress.

  • Skills:

  • Strong understanding of trauma-informed care and youth development practices.

  • Excellent interpersonal skills and the ability to build trusting relationships with youth facing challenging circumstances.

  • Ability to work independently, manage time effectively, and balance multiple priorities.

  • Strong communication and collaboration skills, with the ability to engage landlords, service providers, and community resources.

  • Organized, detail-oriented, and capable of maintaining accurate records and documentation.
